Forma Actuarial Consulting Services, Inc. (Forma ACS) is an independent, Minnesota-based actuarial consulting firm, founded in 2010, specializing in strategic health care consulting, financial modeling, data analytics and actuarial valuation.
Forma ACS consults with health plans, providers, integrated delivery systems, employers and government entities; providing project management and analytic services to meet a wide variety of client needs. Forma ACS is operated and managed by Mike Schoeberl, FSA, MAAA, who is the sole practitioner of the organization.

Mike Schoeberl, Forma ACS founder and president is a Fellow of the Society of Actuaries and a Member of the American Academy of Actuaries. He is also Group Benefits Associate (GBA) of the International Foundation of Employee Benefit Plans. He has worked as a consulting actuary for over 20 years.
Mike has extensive employer, health plan and provider consulting experience, and specializes in strategic plan design, provider performance analysis, financial modeling, data management and risk management. As an expert in risk adjustment, he has played a major role in the development of ACO risk-sharing models and tiered provider networks. He has been a speaker at numerous professional conferences on the use of risk-adjustment in provider performance analysis and cost forecasting. He also has significant experience in member attribution, target development, risk agreements, and other topics relevant to ACOs and provider-owned plans. Mike has led a wide variety of projects requiring the management and use of claims data to help clients understand their past and current status, identify their strategic options, assess the potential impact of decisions and monitor their results.
